How do I find samples of survey questionnaires with the questions used and all?

Answer

Depending on the intended audience, the purpose, etc. and whether you are looking for a commercially published test/survey or an unpublished one (also called experimental), you may need to look in library databases or through the open web. 

Some research articles may provide the full text of the survey within the article (in the appendix). Please see the attached guide on using the Tests & Measures Field in the PsycARTICLES & PsycINFO database. 

Many commercially published surveys will not be availble for free. See the link to the Tests, Surveys, & Measurements Guide for additional resources including where to find full text surveys online. 

Dissertations may be a source for full text surveys though these will often be considrerd experimental ones. You can search the Digital Dissertations and Theses database through the library by clicking on FIND NOW under Find Dissertations on the library homepage.
